Use of Smile Perfecto
I understand that Smile Perfecto are cosmetic, clear, teeth aligners, designed to help improve my smile without seeing a dentist or medical doctor. By entering into this Agreement, I represent that I regularly see my own dental care professional and have no untreated dental conditions; I will continue to see a dentist while undergoing this cosmetic enhancement. I understand that unlike traditional braces, Smile Perfecto are custom trays that may improve and brighten smiles at a lower cost than traditional braces. I instruct Smile Perfecto to fabricate a series of clear aligners that will cosmetically enhance my smile. By purchasing the Smile Perfecto Aligner Kit, I am agreeing to these terms and conditions.
I understand that Smile Perfecto are orthodontic, clear, teeth aligners designed to help improve my smile under the supervision of a dentist or orthodontist licensed in my home state. By entering into this Agreement, I represent that I regularly see my own dental care professional and have no untreated dental conditions; I will continue to see a dentist while undergoing this cosmetic/dental enhancement. I understand that unlike traditional braces, Smile Perfecto Aligners are custom trays that may improve and brighten smiles at a lower cost than traditional braces. I instruct Smile Perfecto to fabricate a series of clear aligners that will cosmetically enhance my smile. By purchasing the Smile Perfecto Aligner Kit, I am agreeing to these terms and conditions.
